Not for free to send it and save to computer. Almost $7 for a year isn't bad though! But I'm not sure if i'll be using this all the time though throughout the year.

I am just getting started. I am happy with the forms - only done one but in a panic- and you all came through. However, I signed up for the monthly plan and I got a bill saying the amount was $180 - the annual amount.How can I verify that I signed up and will be billed monthly at $15.00? I will be happy to do a survey in a month or two.

I find it so much easier to edit documents, and save for later. Being able to share and easily find them. As well as changing page lay outs for presentations too.

I used the trial to fill out an application, but had no reason to subscribe further. The software is easy to use and very useful. I attempted to cancel auto-renewal but i got an email saying I was charged for a year subscription. After contacting customer service, the charge was cancelled within 5 minutes. I was impressed with how quickly and thoroughly they responded to my issues!

Do medical records change numbers?
No. The medical record number, like your chart, is unique to that medical facility or hospital system. Each Hospital has their own unique numbering system.
Are medical record numbers universal?
All health records have financial records attached. But more important, history shows that universal IDs are always used in unintended ways. Social Security numbers were to be used only for payroll taxes, but morphed into universal IDs for health and commerce. UPI's will share the same fate.
