I don't know why each time I do a function, I have to go back to MY Forms. It would be easier to save or print and stay where I am.
Also, when I update a form, the original creation date stays instead of the updated date.

What forms of ID do you need to change your name?
Legal Document: a certified marriage certificate, final decree of divorce, or name change judgment from the court is needed. Identification: driver's license, state ID card, naturalization certificate, or passport with your birth name. Date of Birth: a birth certificate, adoption record, or hospital record.
Can I change my gender on my ID?
Different procedures and requirements for legal name changes and gender marker changes on birth certificates, drivers licenses, social security identification and passports exist and can be inconsistent. Many states require sex reassignment surgery to change their name and gender marker.
How do I change my name and gender marker?
To update your legal name, submit documentation of legal name change (typically court order) To update your gender marker, submit a completed Gender Category Request form (indicating designation of M, F, or X) Pay the application fee. Get a fingerprint scan.
How do you legally change your name and gender?
