Just needed access to a quick fillable PDF for SOAP notes when my office management software with builtin SOAP notes crashed. It is filling a need for the moment, doubtful I will continue. It seems like a nice program but the diagram on the SOAP notes is too cumbersome to mark with the given symbols on the chart as, so far, I have seen no way to actually mark the figure on the chart with the same symbols electronically. There are some symbols available, like the check, circle and line but they are not like the ones on the actual symbol chart. If there was a more expedient way to use those same symbols I might actually be very interested in using this.

To remove cell formatting in Excel, select the cells from which you want to remove all the formatting. Then click the Home tab in the Ribbon. Then click the Clear button in the Editing button group. Finally, select the Clear Formats command from the drop-down menu that appears.
Just select the cells you'd like to delete, then press the delete key. Notice that deleting cells this way removes the data but not the formatting. If you want to remove the formatting, use “Clear all” in the Clear menu on the home ribbon. Another easy way remove data in a worksheet is to delete entire columns or rows.
To remove cell formatting in Excel, select the cells from which you want to remove all the formatting. Then click the Home tab in the Ribbon. Then click the Clear button in the Editing button group. Finally, select the Clear Formats command from the drop-down menu that appears.
Clearing cells means erasing everything within them, whereas deleting actually deletes the entire cell (or cells). To clear the contents of cells you have selected, display the Home tab of the ribbon and click the Clear tool in the Editing group.
Select the data: Go to the Home tab and on the right side click the white eraser and select the Clear Formats option(you can also use the keyboard shortcut Alt + E + A + F and this shortcut also works in Excel 2003): Now, your data is format-free:
Press Ctrl-Spacebar You can also clear the formatting using a simple keyboard command.
On the Inquire tab, click Clean Excess Cell Formatting. Choose whether to clean only the active worksheet or all worksheets. After excess formatting has been cleared, click Yes to save changes to the sheets or Now to cancel.
