The questionnaire asks applicant to send proof of income and supporting documents for all expenses. I do not know how to send these and hope I'll hear from someone about how to do this.

I'M disabled, and confined to a wheelchair. I'm not able to get out, and look for a place that will agree to fax things. I have arthritis real bad, and it is painful to hold a pen, and fill things out. The ability to do all of this for my nurses, and care-provider's, with PDF filler online is a true blessing. VERY EASY TO USE. THE ONLY THING I THINK NEEDS TO CHANGE IS THAT WHEN I SAVE THE DOCUMENT, INSTEAD OF SAYING IRS FORM 940 (OR SOMETHING CLOSE TO THAT) I THINK THE YEAR OF THE FORM THAT WAS WORKED ON. FOR EXAMPLE: IRS FORM 940 2019
