trying to find the right form that was not already filled out.
Also you need to add the ability to erase to the lowest membership level. You can do a couple of things such as restrict the number of new forms per month, offer a 1-5 form only fee, or only a 1 month membership fee. In the past 3 years I've used this service only twice now as I am not a business.

I tried it before purchasing. The font, tick box selection and general feel is better, in my opinion, than that I have experienced with Adobe. My abilities are significantly challenged due to illness. The PDF document that I have to use presents with problems when I run it in other edit utilities. I may call on you if I get stuck, but so far I am favorably impressed with this product. Thanks. ;-)

How do you make radio buttons?
Begin by creating an input element to serve as the basic foundation.
Set the type to radio.
Give each radio button a unique id.
Use the name attribute to identify all the buttons in a group.
Consider visual grouping as well.
Should radio button be selected by default?
Give people control and align with their expectations (Good): It is better to have a selected radio button by default, given that people cannot deselect and set the button back to its original state once one has been selected. A default selection sets the correct user expectation.
How do you add radio buttons?
To create each radio button option, create a RadioButton in your layout. However, because radio buttons are mutually exclusive, you must group them together inside a Radiogram . By grouping them together, the system ensures that only one radio button can be selected at a time.

Only one radio button in a given group can be selected at the same time. Radio buttons are typically rendered as small circles, which are filled or highlighted when selected.

In case of radio buttons this means that radio buttons should always have exactly one option preselected.
