I didn't realize how much use I would…
I didn't realize how much use I would make of this program, or the range of functions that it offers. I just got the free-version at first because I needed a digital signature. But now, as an academic, I find I use it all the time to share and to sign documents: now that everything is online and we have limited contact, this has become more important. I can't say that I've mastered or even noticed all the file features this program offers, but almost every time I use it I discover something new. Well worth the money!

How do I print my chart?
Internet Explorer: Select File in the upper left corner of the screen, and then select Print. Chrome: Select the three stacked dots the upper right corner of the screen, and then select Print.
How do I print my Chart results?
Then look in the upper right-hand corner of the screen while the result is open. You will see a small printer icon. Click on that icon, and you can then print the results. This 'printer' icon is also used on other screens to print information from Chart.
How do I find my activation code for my chart?
Please fill out the online Activation Code Proxy Request form. You will receive your Chart activation code in the mail within 7 to 10 business days. Once you activate your Chart account, you can then view your child's account by clicking on the tab with their name (on the right side of screen).
How do I download my Chart app?
Access the App Store or Android Market on your mobile device. Search for 'Chart' to locate the application. Download the free Chart application. Launch the Chart application and select Cleveland Clinic as your healthcare provider. Log in using your Chart username and password.
How do I link my two Chart accounts?
To request access to another account, log into Chart and then select the Request access to another account (proxy) link under Quick Links located near the upper right-hand corner of the main Chart page. Follow the instructions.

Chart offers patients personalized and secure online access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With Chart, you can use the Internet to: Communicate electronically and securely with your medical care team.
How do I get on Chart?
Your Chart information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.
How do I sign up for Chart?
Activate Your Chart Account Enter your Chart Access Code exactly as it appears on your After Visit Summary or enrollment letter. You will not need to use this code after you've completed the sign up process. Please enter the last 4 digits of your Social Security Number.
