Works fast and easy, great service!I had signed up very briefly to edit some pdfs for work. The program worked smoothly, but then I foolishly forgot to end my membership, and was billed for a years' worth of their service, which I did not need. Their customer support was top-notch and had my problem resolved in just a few minutes. I'd recommend them for anyone looking for a company that is responsive to their customers.

What is the proper way to make changes to a written health record?
Draw line through entry (thin pen line). Make sure that the inaccurate information is still legible. Initial and date the entry. State the reason for the error (i.e. in the margin or above the note if room). Document the correct information.
Is it illegal to change medical records?
Is it Illegal to Alter Medical Records? Altering a medical record is a crime and can also be used against doctors in medical malpractice cases. However, it is not illegal for medical professionals to make honest updates to records, as long as they properly mark what they are doing and do not obscure information.
Is falsifying medical records a felony?
Falsification of a medical record with any kind of alteration or destruction is considered as tampering with evidence in a medical malpractice case. Finally, knowingly falsifying medical records is a felony crime with a potential fine of $250,000 or five years in prison.
Can you change a medical record?
If you think the information in your medical or billing record is incorrect, you can request a change, or amendment, to your record. The health care provider or health plan must respond to your request. If it created the information, it must amend inaccurate or incomplete information.
Can you have a diagnosis removed from medical records?
Individuals may want the initial diagnosis to be deleted on the grounds that it was, or proved to be, inaccurate. However, if the patient's records accurately reflect the doctor's diagnosis at the time, the records are not inaccurate, because they accurately reflect a particular doctor's opinion at a particular time.
Can you sue a doctor for falsifying medical records?
Falsifying medical records is not necessarily grounds for a medical malpractice lawsuit, but may be grounds for an independent civil action for fraudulent concealment or spoliation of evidence.
When should a medical record be changed?
Your provider must act on your request for an amendment no later than 60 days after receipt but may extend by 30 days if a reason for the delay is provided in writing. If your provider does not provide a reason, they must amend the inaccurate or incomplete information. There are a few exceptions.
How long does your office have to respond to a patient's request to amend a record?
The patient should submit a written request to amend the health record. The provider has 60 days to respond with written notification, and may extend the time frame an additional 30 days, if necessary.
