This is the faculty member who you will meet with one on one or in class during or prior to your internship. Your faculty supervisor is not the faculty member who assists in registration for the classes. Next, fill in your name and contact information. If you are a double major select the college or professional school that is affiliated with your primary major. These next questions are related to the academic credit aspect of your internship. Please fill in your faculty supervisor information the number of credits you are receiving, the course number, and the semester that the majority of the internship is completed within. Research has shown that interns who establish clear learning goals prior to starting an internship have a well-rounded, successful experience with higher performance. Please list four to six goals for your internship. Progress on and completion of these learning goal objectives will assessed during the midpoint and final evaluations. So it's important that these goals really matter to you.
