If you are not receiving credit for your internship please see the tutorial for completing an internship that is not for credit. By completing this form you'll provide us with information on how to best support you during your internship. Your involvement in an internship helps University of Portland to better understand our community partners and provides opportunity to promote and build on internships for current and future students. Involvement in an internship also speaks to the success of UP students, and we want to share the great work that our students are doing. Once you click on the link for the University of Portland Internship Learning Agreement. You'll be prompted to begin the form online. Since your internship is being completed for credit, please select 'yes' under the question 'Are you completing your internship for academic credit'? You will then select the primary program which is awarding you your internship credit. If your internship is involved in two internship programs please select the program that is aligned with the primary faculty supervisor for your internship. This is the faculty member who you will meet with one on one or in class during or prior to your internship. Your faculty supervisor is not the faculty member who assists in registration for the classes. Next, fill in your name and contact information. Make sure that spelling is correct especially your email. Then you will be prompted to fill in information regarding your major and your class year you are completing the internship. If you are a double major select the college or professional school that is affiliated with your primary major. These next questions are related to the academic credit aspect of your internship. Please fill in your faculty supervisor information the number of credits you are receiving, the course number, and the semester that the majority of the internship is completed within. Research has shown that interns who establish clear learning goals prior to starting an internship have a well-rounded, successful experience with higher performance. Please list four to six goals for your internship. Progress on and completion of these learning goal objectives will assessed during the midpoint and final evaluations. So it's important that these goals really matter to you. If you need assistance in crafting your learning goals, please visit the career center webpage at www.up.edu/career/internship or call the career center at 503-943-7201. You will then need to complete some information about internship supervisor. This is the supervisor who you will work with at the internship site. Please list his or her name, the internship organization name, and the internship supervisor's email address.
