Involvement in an internship also speaks to the success of UP students, and we want to share the great work that our students are doing. You will then select the primary program which is awarding you your internship credit. If your internship is involved in two internship programs please select the program that is aligned with the primary faculty supervisor for your internship. This is the faculty member who you will meet with one on one or in class during or prior to your internship. Your faculty supervisor is not the faculty member who assists in registration for the classes. Next, fill in your name and contact information. If you are a double major select the college or professional school that is affiliated with your primary major. These next questions are related to the academic credit aspect of your internship. Research has shown that interns who establish clear learning goals prior to starting an internship have a well-rounded, successful experience with higher performance. Please list four to six goals for your internship. Progress on and completion of these learning goal objectives will assessed during the midpoint and final evaluations. So it's important that these goals really matter to you. You will then need to complete some information about internship supervisor. This is the supervisor who you will work with at the internship site.
