It's an awesome program. The only problem is that this form is used with all insurance companies and the top should be blank for that particular company to fill in their name.

For pdfFiller’s FAQs

Below is a list of the most common customer questions. If you can’t find an answer to your question, please don’t hesitate to reach out to us.
Write your signature on a neutral-colored sheet of paper with a black pen. Scan the signed sheet at a resolution of 300 dpi. Save the scanned file as a GIF, JPEG, TIFF file. Import the scanned signature image into a photo editing program like Photoshop, Gimp, Paint or Illustrator.
But the thing is this signature is always the same and that means it is the same length. This is why when you sign your photos you should always sign on the right-hand side as there is no risk of the signature going past the right edge of the photo, as doing this is and looks awful and unprofessional.
In general, landscape prints get signed and portrait prints don't. However, it is a pretty common practice for portrait photographers to put a small digital signature on their prints at about 30% opacity in the bottom corner.
Docker is a platform that runs each and every application segregated and securely by the use of kernel containerization feature. Docker Image is a set of files which has no state, whereas Docker Container is the instantiation of Docker Image. In other words, Docker Container is the run time instance of images.
Docker is a tool designed to make it easier to create, deploy, and run applications by using containers. Containers allow a developer to package up an application with all the parts it needs, such as libraries and other dependencies, and deploy it as one package.
Docker Content Trust is a feature in the Docker containerization platform that enables remote registry content to be digitally signed, ensuring that the content is unaltered and is the most current available version when users access it. It works via cryptographic keys.
Docker Trusted Registry (DR) is the enterprise-grade image storage solution from Docker. You install it behind your firewall so that you can securely store and manage the Docker images you use in your applications.
A Docker repository is where you can store 1 or more versions of a specific Docker image. An image can have 1 or more versions (tags). It's also worth pointing out that the Docker Hub and other third party repository hosting services are called registries. A registry stores a collection of repositories.
The store that actually contains those assets is the repository. A repository, like the gold repository in Fort Knox, stores actual assets and typically controls access to those assets. So, while a registry simply records official information that relates to an asset, the repository stores the assets themselves.
Pushing an image to Docker Trusted Registry is the same as pushing an image to Docker Hub. Since DR is secure by default, you need to create the image repository before being able to push the image to DR. In this example, we'll create the 'golang' repository in DR, and push the Golang 1.7 image to it.
More About Docker Hub As we've mentioned, Docker Hub is just one of the registry providers. And a good one at that. We can find all sorts of images over there and push our own. We can create unlimited public repositories and one private repo free of charge.
Docker Hub is a hosted repository service provided by Docker for finding and sharing container images with your team. Key features include: Private Repositories: Push and pull container images. Automated Builds: Automatically build container images from GitHub and Bitbucket and push them to Docker Hub.
