After contacting customer support, I was told that certain web browsers are more compatible than others in completing forms. Once I changed from Mozilla Firefox to Chrome, I was able to complete all fields of the form without problems. But it would have been helpful to publicize this information on your website so that much time was not wasted.

Adapting to Monovision Contacts It usually takes at least 1 to 2 weeks to adapt to monovision. The vision will continue to improve as the lenses are worn.
Although monovision does not work for everyone, it is a viable option for people who have presbyopia. Monovision can help you achieve good, functional vision at distance and near. As a result, patients who require the sharpest corrected vision possible at distance or near may not be the best candidates for monovision.
While Monovision is not a perfect solution to presbyopia, for carefully selected patients, it is well tolerated and very satisfactory over 85% of the time. Most patients who choose Monovision are satisfied with both near and far vision without glasses.
The downside of monovision is that some people find it compromises the clarity of their distance vision too much, making distant objects appear slightly blurred. Others find monovision doesn't provide adequate near vision to give them the freedom from reading glasses they were hoping for.
Of these patients, 67% chose to be corrected for monovision, with women outnumbering men 60% to 40%. While most of these patients chose their dominant eye to be corrected for distance, investigators found that those who had this eye corrected for near achieved similar visual function.
Monovision contacts correct one eye for distance vision and the other for close-up vision. If you do not correct presbyopia, you may be bothered by headaches and eye strain.
