My bank required my signature not an electronic signature. Maybe if I used my signature on pdf format I will try that next time.

The site is very easy to navigate. I'm a novice when it comes to anything to do with forms, etc. Your site is so easy because it's intuitive. It just makes sense about what to do next, no hunting around trying to figure what the site builders were thinking. Easy peasy.

So far my overall experience has been pretty great. The only thing I would recommend would be having more of a collection of pdf's to compare on certain subjects. Other than that, I am one happy customer!

This morning my subscription to PdfFiller was automatically renewed. Once I realized it, I notified the company to cancel my subscription and to provide a refund. I can manage various professional documents with minimal difficulty---especially during these past few years, when electronic submissions replaced face to face meetings.

Nothing, really. All of the features are clear and easy to use.

Does United Healthcare require pre-authorization?
1, 2017, notification is required for certain genetic and molecular lab tests for certain UnitedHealthcare Commercial benefit plans. Ordering care providers will complete the notification/prior authorization process online or over the phone.

How do I get a prior authorization for UnitedHealthcare?
UnitedHealthcare prior authorization forms If you cannot submit requests through electronic prior authorization (EPA), or for urgent requests, please call us at 1-800-711-4555. Otherwise, you can submit requests to the Optimal® Prior Authorization Department by completing and faxing the applicable form.
Does Medicare require pre-authorization for MRI?
In order for an MRI (magnetic resonance imaging) scan to be covered by Medicare, it must be ordered by your doctor or other health care provider who determines the procedure to be medically necessary as part of your treatment plan.
Does United Healthcare require pre-authorization for physical therapy?
For dates of service based on the member's benefit plan type, we require prior authorization for outpatient physical, occupational and speech therapy services for all UnitedHealthcare Community Plan of New York members. Claims will be denied if prior authorization is not on file before the date of service.
Does United Healthcare require a referral for physical therapy?
Not all health plans require a referral, but if your plan does, ask your PCP or clinic for an electronic referral before you visit a specialist. If you don't know if you need a referral, sign in at myuhc.com > Coverage & Benefits or call the number on your health plan ID card to find out.
