Why is my form unavailable on Jotform?
It seems your form is currently disabled, which is why you are getting the error message 'This form is currently unavailable'. Enabling the form again is easy let me show you how: Open your form in Form Builder and click on the Settings tab. Go to Form Status, and make sure that the status is Enabled.

The Date field lets people enter a date in the proper date format, or choose a date via a date picker calendar.
