I cannot get this to print correctly; all I am getting aare dots on form.

Why you received IRS Notice CP01A You notified the IRS of your identity theft and an ID theft indicator was placed on your account. The IRS sent the CP01A to provide you with an Identity Protection Personal Identification Number (IP PIN) for you to record on your next tax return.
What does cp01a notice look like?
This notice tells you about the Identity Protection Personal Identification Number (IP PIN) we sent you. Each year, we issue CP01A notices that have a unique 6-digit IP PIN with instructions on how to use it.
When should I receive my IRS pin?
Retrieve your IP PIN online at the IRS's Get an IP PIN site. Or. Call the IRS at 1-800-908-4490 to have your IP PIN mailed to you (this can take up to 21 days)
Does IRS IP PIN change every year?
A1: The IRS IP PIN is a 6-digit number assigned to eligible taxpayers to help prevent the misuse of their Social Security number on fraudulent federal income tax returns. A new IP PIN will be generated each year. This includes current year returns as well as any delinquent tax returns.
Can you file taxes without IP PIN?
Without a new IP PIN, taxpayers will have issues filing their federal tax return. They will have to file their tax return on paper, by mail, as the IRS will reject any electronic version submitted without the proper IP PIN.
Does everyone get a cp01a notice?
Each year eligible taxpayers should receive a new CP01A Notice (and new unique number), and the Identity Protection PIN that the taxpayer received last year will not be valid for filing their 2017 tax return.
