I used the trial version of PDFfiller… I used the trial version of PDFfiller for 30 days and it did exactly what I needed. I intended to cancel my subscription on day 30 before I was charged because at the moment I only needed it for one task. They charged me on day 30 but I emailed support and they immediately refunded my money with utmost courtesy. Overall very handy. I did have a glitch in printing. When I hit Print Again, it made the page smaller and off center. This happened each time I hit Print Again on the CMS 1500 form, requiring me to go back and start from Print after each edit.

Customer services are A++, an annual subscription fee was taken as I had forgotten to stop this after my use, upon emailing pdfFiller's 'Live chat' the refund was processed and I received a confirmation email of this from PayPal in less than 30 seconds! If it wasn't for Covid19, I would have continued the subscription, but after losing my job my finances are paramount! Hopefully I will be in a position to return to your service in future!
