Recently I needed to fill out several forms from my mortgage servicer. They emailed the forms in pdf format and wanted them signed and returned same day. I don't have a scanner available so I googled and found PDFfiller. I was able to get my forms filled out, signed, and sent back in a very short time. PDFfiller is very easy to use. It really saved my day!

PS: Hold down Ctrl key to uncheck. Radio buttons are meant to be used in groups, as defined by their sharing the same name attribute. Then clicking on one of them deselects the currently selected one.
You can deselect options in a multi-value select list by ctrl-clicking it.
If a radio button is unchecked then a user can check it by simply clicking on it. Once a Radiation is checked by user it can't be unchecked by simply pressing on the same button. It will automatically uncheck when you press any other RadioButton within same Radiogram.
Radio buttons are groups of buttons in which, by convention, only one button at a time can be selected. The Swing release supports radio buttons with the JRadioButton and ButtonGroup classes. To put a radio button in a menu, use the JRadioButtonMenuItem class.
The best way to align radio buttons is using the vertical align middle CSS with margins adjusted on the input elements. I like putting the inputs inside the labels (added bonus: now you don't need the for attribute on the label), and put vertical-align: middle on the input. (The -2px margin-top is a matter of taste.)
To make a horizontal radio button set, add the data-type=”horizontal" to the fieldset . The framework will float the labels, so they sit side-by-side on a line, hide the radio button icons and only round the left and right edges of the group.
Wrap your form in a div. Set the div's display to block and text-align to center (this will center the contained form). Set the form's display to inline-block (auto-sizes to content), left and right margins to auto (centers it horizontally), and text-align to left (or else its children will be center-aligned too).
baseline This is the default value. top Align the top of the element and its descendants with the top of the entire line. bottom Align the bottom of the element and its descendants with the bottom of the entire line. middle Aligns the middle of the element with the middle of lowercase letters in the parent.
The CSS just sizes the div, vertically center aligns the span by setting the div's line-height equal to its height, and makes the span an inline-block with vertical-align: middle. Then it sets the line-height back to normal for the span, so its contents will flow naturally inside the block.
