What is the purpose of the DA Form 5118?
The first purpose of the DA Form 5118 is to carry out the initial screening of reassignment cycle to determine a soldier’s eligibility to comply. Secondly, it is used as basis for commencing specific assignment processing (for instance, additional service; deferment/deletion; or any other special processing that may be required).
Who fills the DA Form 5118?
The DA Form 5118 consists of four parts. The Military Personnel Division/Personnel Service Company answers the questions in Part I. Before answering the questions, the aforementioned authority has to compare the soldier’s Personnel Qualification Record with the ERAS Cycle.
The Battalion S1 answers the questions in Part II, signs the completed statement and returns it to the Military Personnel Division/Personnel Service Company along with completed Soldier Status and Election Statement.
The soldier answers the questions in Part III and Part IV of the form.
Is the DA Form 5118 accompanied by other forms?
No other form accompanies the DA Form 5118.
When is the DA Form 5118 due?
The form is used as soon as the need arises. The Personnel Reassignment Work Center uses it to determine (1) whether a soldier or an officer requires an updated personnel security investigation (PSI), SBB or Personnel Reliability Program or Chemical Personnel Reliability Program screening; (2) whether a soldier should acquire any additional service to complete prescribed tour.
What information should be provided?
The DA Form 5118 contains more than 80 questions.
The Military Personnel Division/Personnel Service Company answers questions about special management command, general eligibility, and time in service requirements' status of the soldier.
The Battalion S1 answers questions regarding the duty status and pending action status.
The soldier answers questions about his personal status, to and from OCO NUS status, and wartime status.
Where do I send the DA Form 5118?
The soldier signs the completed statement and returns it to the Battalion S1.
