You are here and in good physical condition with your own transportation. I agree to the Terms and Conditions and I understand that it is my responsibility to attend the classes. I understand that Participant and Host will need to register for the program as a Host as well as a Participant with their own transportation. Furthermore, I understand that Participant and Host must both attend the program at least 2 times each week. Furthermore, I agree that I understand the Terms and Conditions and accept them and am eligible for this program. Furthermore, I understand that Participant and Host may both attend only one program per week per group, and may only use their own transportation. Furthermore, I understand that Participant will be responsible for all expenses incurred by the Host in attending the program other than Participant-host transportation costs. For the purposes of this Participant will be referred as Host and Host will be referred to as Participant. Participant I understand that for a host to be eligible for this program and be responsible for all transportation and program expenses they must attend both the host-only program on Wednesdays and the program on Thursdays that are designated only for Hosts. Host should keep a copy of the host's Driver's License until completion of the Trips as proof of registration. This Host is responsible for paying any and all Transportation and Program expenses incurred by the Host for the program(s). I understand that the only difference between this program and a regular program is that host-only class attendance is required; however, this Participant agrees to be responsible for all transportation and program expenses incurred by Host. I understand that hosts MUST attend a program only once per week. Host I have the capacity to travel to all three of our Program classes and that if I am unable to attend all three classes for some reason then host's will be held at a different host location.
