Microbial. 51 : In order to study and understand the effect of PEEP on the intravascular circulation, we have performed high-resolution magnetic resonance imaging (MRI) of a normal control patient, which revealed an increase in blood perfusion volume and volume of the pulmonary vasculature by 16%, and marked vascular and systemic changes within the respiratory muscle. The pulmonary capillary refill rate increased by 12% in response to PEEP by the normal subject and decreased by 20% to 18% in the PEEP + PEEP vs control patients. The heart rate normalized by the PEEP + PEEP in both the pulmonary and systemic regions increased by 14% to 22%. As noted in, during a PEEP-induced increase in the blood perfusion volume, a significant increase of the pulmonary capillary refill rate was observed, indicating a marked improvement of perfusion in the pulmonary vasculature. The systemic vasculature is considered to provide oxygen transportation to the lungs, so this response should be attributed to an improvement in the pulmonary circulation. It is tempting to speculate that this response might be directly related to a more favorable hemodynamic status in the pulmonary vasculature. Since oxygen can be delivered to the brain via the veins into the central nervous system, it is possible that the increase in the pulmonary blood supply also allows increased delivery of oxygen to the brain. This hypothesis requires further investigation. Table 1 Open in a separate window. DISCUSSION It has long been known that blood flow to the lungs is important for respiratory function [1–3]. The main problem with current experimental techniques of pulmonary perfusion imaging is that the hemodynamic parameters are difficult to monitor and are dependent to a large extent on the physiological characteristics of the individual study participants. Several recent studies have improved the performance of current imaging techniques, including, MRI [4–6], CT [7–10], ultrasound [11] and PEEP [12, 13]. However, PEEP has recently been shown to have a large positive effect on the pulmonary vasculature [14–16] and to change the hemodynamic response to blood flow changes [15, 17].
