This form is required by the Headquarters Army Continuing Education System (HQ ACES). The document is filled out by the soldier in case he drops a class. The soldier completes the form and sends it to the commander for signing.
What is the purpose of the Request for TA Recoupment Waiver?
This request is used to provide documentation to verify non-completion of a course due to military reasons for which a soldier received Army Tuition Assistance. A waiver from classes must be approved by the soldier’s commander based on unanticipated military duties or deployment, separation from service, emergency leave (for example, a death in the family), illness, hospitalization, etc.
What documents must accompany the Request for TA Recoupment Waiver?
This form should be accompanied by additional documents if required.
When is the Request for TA Recoupment Waiver due?
The form is to be completed by the soldier when he drops a class during training courses. The approved request will appear in the soldier’s record within 60 days after withdrawal. The Education Services Officer must approve the form to cancel the debt. If the form is disapproved, the TA funds will be recouped.
What information should be provided in the Request for TA Recoupment Waiver?
While completing the form, the soldier has to provide the following information:
- Last and first name
- Rank
- Last 4 digits of Social Security Number
- Signature and date
- Name of the school
- Course and course title
- Start and end date of the withdrawal
- Cost
The soldier also has to provide the details of the reason for the unanticipated withdrawal. If additional space is needed, the soldier must use a separate sheet.
What do I do with the form after its completion?
The completed request is forwarded to the soldier’s commander for signing and consideration.
