The referral helps coordinate the member's care and ensures they receive the necessary resources and expertise to address their health conditions effectively.
The person who is required to file a UHC Community Plan Referral would depend on the specific circumstances and policies of the UHC Community Plan. In general, an individual seeking a healthcare service that requires a referral may need to initiate the referral process themselves, while others may require a healthcare provider, such as a primary care physician, to submit the referral on behalf of the patient. It is advisable to directly contact the UHC Community Plan or review their policies to determine the exact requirements for filing a referral.
