Application Eligibility Information: Applicant should have a permanent mailing address and should be able to provide proof of current address. Applicant's federal, state, and local tax identification numbers (Social Security numbers) will be checked against the National File and will be checked against a prior year's Federal Homeownership and Equity Protection List. For verification purposes, the applicant must provide a letter from the IRS, IRS-NFS, the Virginia State Treasurer, or the local Virginia State Tax Collector affirming that all requirements for listing on the Federal List have been met and that the Applicant has met all other applicable eligibility requirements. The applicant should obtain a valid and current Virginia Driver's License or Virginia identification card which shows the Applicant's permanent mailing address. If a resident does not already have a valid Virginia Driver's License or Virginia identification card, the applicant should obtain prior to completing the SHOP Program application an annual Virginia State driver's license, identification card, Virginia vehicle registration, or Virginia school enrollment card from their local DMV office. The applicant should include, in their application for eligibility, the current and prior year Federal Deposit Insurance Corporation (FDIC) and/or State unemployment insurance claims. For a list of available insurance, please call the Virginia State Department of Revenue/Office of Insurance and Financial Services at (during normal business hours).
