Enrollment Information from Other States
Some plan providers use enrollment information from other states to verify plan coverage for their subscribers. As long as the state provides proof that you are entitled to benefits under the plan, that is enough to enroll in the plan. In doing so, the other state will not be able to verify that you are not a current Health Net customer and will not be able to use your health status for administrative purposes.
To be sure, you must have obtained the enrolled agreement. The agreement will provide you with the name/address of your health plan provider in all the states in which you are enrolled. In the enrollment agreement, the other states' health plan plans must be listed. Please note that the information you list to Health Net in the agreement applies to your individual plan. Plan enrollment information from other states is not accepted for any purposes.

Information from a social security account or Medicare Part D benefit number
Information from a State government website
A federal or state government website
Medicare or Medicaid Number
The enrollment information from other states will not be accepted. If you receive a response from an enrollment site other than Health Net indicating that some or all of your information was received via that site, please contact us as soon as possible.
