There are several areas where you should make corrections. As a result, the information in the following section will be updated to show appropriate corrections. If you have completed the “Employer Name, Employer Corporation, and Employee's Name” and “Employer Corporation” sections of the form, you should make the following corrections: Employer's name must be listed on line 5b, Employer Corporation name on line 9, and Employer's Name, company name and the corporation name on line 11. If the business name of the employee is not listed in lines 5b, 9, and 11, the following correction is required. The first letter of the company name should not be a capital “C”. If the first letter is a capital “C”, add a space, followed by the employee's name and their business name. Do not re-submit Form RR-2A-2 if the company name on line 11 is missing. The correct address on line 11 is 1?18 E. North Ave., Springfield, IL 62702. Employee's name: (Employer Corporation) should be the first word of the employee's name on line 11. Employer's name and employer corporation should be listed in the middle of the form. Employee's first name (first name and their last name) should be the first word on line 11 of the form. Employees first names should not be capitalized. If the first letter of the employer names is a capital “C”, then the word “employer” should be capitalized, followed by the form number, followed by the first word of the employee's last name, followed by the rest of the employee's last name, last two. If there are letters between the employee's first name and last name, then remove all but the last letter or the first letter and capitalize “employer”.
