However, the individual cannot apply for a waiver by using the form. Rather, the waiver process would require the individual to submit a Military Disqualification form (Form 302). The individual can also use the waiver form to obtain an administrative allowance, as well as to apply for a special or permanent disability pension to meet any remaining financial obligations that the person may have, and to recover costs associated with service or rehabilitation in order to regain the ability to work. Issue 2.1 — Medical Waiver forms (PDF) are required for an assessment of medical conditions and have been adopted by the Canadian Armed Forces. Waiver forms and instructions on how to complete the forms are included in Form 302 that is provided in the Personal Application (PC) for a Military Disability (PDF) (Form 302/PC — Form 302.1). The PC allows a member of the Canadian Armed Forces to submit and be approved a waiver form for a medically-based medical condition (Medical Waiver). A Medical Waiver (Form 302) must contain instructions on how to complete the form or how to return it. Some medical conditions do not need medical waivers, however, for some conditions, such as a mental disease or an abnormality that causes mental deterioration such as seizures, a medical waiver is required. It is recommended that members be checked in to their local military health center on their first day at work and provided with the form if they fall into a category of service members that has these requirements, such as the Canadian Expeditionary Force. The medical waiver form must be completed and stamped by the nearest Canadian Armed Forces Health Clinic, or an authorized medical provider designated by the Canadian Armed Forces and issued a clearance certificate. If a medical waiver form is not filed, then the Canadian Armed Forces will determine whether the member has the requirement for a medical waiver.
