Methods We conducted a population-based study, including questionnaires and interviews involving 18,315 participants born in the 1990s with an initial survey in 1997 of the residents between the age of 20–79 years in Brooke County and their parents. The study was conducted from July 2004 through February 2005 and was approved by the Village Administrator of Village of Endicott. Result Results from the 2005 study are consistent with those from earlier studies in terms of the prevalence of a lifetime exposure to uranium, as well as the associations between these exposures and incident cancers. Most participants reported a lifetime intake of more than 0.03 g/day from uranium. Median cumulative exposure during a lifetime to the highest versus lowest quartile of uranium was 0.01 mg/day for the general population, 0.002 mg/day for workers, and 0.008 mg/day for the residents. Lifetime exposure to uranium was significantly related to a higher risk of a first-mortality-related cancer among those with a lifetime intake of less than 0.1 g/day, for men vs women, and for all cancers regardless of age at the time of exposure, but was not significantly related to a rate of mortality-related cancer for individuals with a lifetime intake of more than 0.03 g/day and the lowest level of exposure. In the general population, higher cumulative exposures to uranium were associated with higher average annual uranium concentrations at birth among men, and the association was slightly stronger among women. In workers, higher average annual uranium concentrations at birth were associated with higher average age at time of exposure, and the association between average annual uranium concentrations at birth and age at time of exposure was stronger among men than women. In the general population, higher cumulative exposures to uranium were associated with a greater rate of first-mortality-related cancer among smokers, in both sexes, and for those in the lower intake groups compared to non-smokers. Significant associations appeared for all cancer types, for each of the intake levels measured and when the uranium intake levels were compared to those estimated from the U.S. Environmental Protection Agency.
