In the last decade, surgical resection of the distal rectum with a conservative approach (i.e., a colostomy, but with abdominal mobilization) has become more popular and is advocated as the future standard approach for distal colon cancer. Although this approach has been studied for a range of colostomy-related conditions, the effects of the technique on disease progression in colostomy-incidence patients have been less well-defined. This article reports the results of a prospective study of the effect of a conservative procedure on disease progression during a distal colostomy. The patients included in the study were treated for distal rectal carcinoma in 1990 and 1991 by colostomy and noncolostomy control surgical resection with local excision. Colorectal resections with excision were used as the primary method of treatment because of the less distal, more respectable nature of cancer. A clinical follow-up questionnaire to assess the impact of the colostomy on disease progression was mailed to the cohort on a yearly basis starting in September 1990. At the conclusion of the study in September 2001, a total of 486 patients were included in the study. The overall annual incidence during follow-up was 4% (95% confidence interval (CI), 1% — 5%). When treated by colostomy, all-cause mortality was 7.2% vs 7.1% in the noncolostomy group.
