The 571-L Business/Personal Business Application (571-L) is a two-page form designed to ensure that the individuals and businesses that make up the County's taxpayers are being fairly represented in their elections. The form is available on this website and is available from our staff in the Registrar/County Clerk office at least four days prior to the election. Please review the information on this form carefully, then complete and return and mail to our office with either the completed form OR a scanned copy of the form. If either the original or the scanned duplicate does not have the signature of a registered voter in the district, it will not be considered valid and will not be accepted. The voter can sign the original form or the scanned copy, and sign the second form OR the second scanned copy. It is your responsibility to ensure that the signature of each voter on the completed forms is in fact their signature and not a duplicate. Once the proper documents have been obtained, it is the responsibility of the voter to keep the completed forms and return them to the registrar/county clerk to insure an accurate and correct signature for each voter on the petition. Each voter is also required to sign, under penalty of perjury, the first, second and third pages of the form under penalty of perjury. This is so the registrar/county clerk can verify with the voter's signature.
