In recent years, there has been growing interest in language-based hearing loss with more than 90% of deaf and hard-of-hearing adults living in a language-hearing network. These individuals have a high need for services, and it is important that service agencies provide them with the services they need. For deaf and hard-of-hearing adults who have low speech volume, there is little agreement on language-based thresholds for defining speech reference. Two studies have examined thresholds in the general population. In each study, the thresholds were defined with various levels of speech reference [4], [5]. These thresholds were used in a variety of different systems and settings. For instance, one study used the Threshold Level for Speech Guidelines [4], the other the Threshold Level for Speech Reference (TLS) for the deaf and hard of hearing adult population in both the U.S. and Australia [5]. Thresholds are used to estimate the amount of background noise, as well as the severity of background noise when the individual is listening to speech. Generally, threshold levels between 7 and 25 decibels are recommended for hearing loss of greater than a 15dB HL loss. In some situations, thresholds can range between 60 and 110 decibels with a few exceptions [1] — [2]. Hearing aids are available to individuals who are deaf or hard of hearing. These aids are used to reduce speech levels in order to reduce the risk of accidents or loss of employment. Hearing aids are available with both spoken and sign modes. Many people can be expected to hear at levels above or below the thresholds used for purposes of this paper. For these purposes, “threshold level” refers to the lowest level of speech reference in which at least 80% of individuals can hear without the use of corrective hearing aids (see Table 1). Although most of the information presented here is based on the speech volume of individuals who have normal hearing, some individual factors may affect the threshold limits. For instance, women tend to have lower average speech volumes than men because of differences in the proportion of hearing-impaired women than the proportion of hearing-impaired men.
