If major is minor, then minor must be listed, and the name of the major must be listed as “minor”. 2. If major is major, then major must be listed, and the name of the major must be listed as the “major”. 3. If minor is major, then minor must be listed, and the name of the minor must be listed as “minor”. 4. If the major or major-minor is combined by “or”, then the resulting field is mandatory. The “or” field is subject to the requirements in #3 below. 5. The major of the student(s) is subject to the requirements in #2 above. 6. The graduate faculty member's name: The name of the graduate faculty member is required. Graduate faculty members should be familiar with the graduate advisor position and the requirements of the graduate program. The name of the graduate faculty member should also appear in a clear, conspicuous manner on the student's graduate transcript or transcript summary. 7. The name of the graduate program: The Graduate Program form will list the major or major and minor of the proposed major. If approved, this will be the field in the Graduate Departmental Information (GDI). If the graduate program is a Ph.D. degree program or a joint degree program, then the GDI may also list the major and/or minor(s). A major or major-minor will be listed as a Ph.D. degree, a joint degree, or a joint degree.d., respectively. 8. The academic year(s) in which the proposed major and major-minor will be completed (e.g., Fall 2008) Graduate students: Academic advisors will not be concerned with students completing a degree from a previous academic year.
