INFORMATION AND SERVICES AFFECTED BY THIS POLICY University personnel who use or disclose Protected Health Information in connection with health care or other research activities must: 1) promptly notify the University's Information Ethics Office of all research involving Protected Health Information in order to determine privacy status and whether the Protected Health Information is subject to an approved security classification; 2) comply with all applicable laws, policies, and procedures pertaining to the protection and use of Protected Health Information; 3) comply with all University policies and procedures for the protection and proper utilization of Protected Health Information; and 4) provide the University's Information Ethics Office with the name of the research sponsor, principal investigator, and study supervisor as well as all other information requested in order to determine compliance with these duties and procedures.
