Larger, editor: NINETY MINUTES A DAY This article reports an unusual case of a 35-Year-Old Woman presented by her psychiatrist as a paranoid schizophrenic who was taking four amphetamine salts and two of the other two, in a somewhat less excessive and with less apparent tolerance to stimulants. Since these substances are usually available from local pharmacies, she asked me to prescribe them. The results are published in a new edition of Neurorehabilitation (issue 2, May 2005). When I arrived at her apartment at about 9.30am my first impression was an intense excitement. She was sitting at the table with her feet up, and she was looking at my face in an excited and happy way. I began to speak to her briefly, asking her what her problems were, she answered that she “was feeling quite good”, I was very interested in what this “feeling” was because I am trying to help to treat patients with paranoia and this woman, despite having a psychotic illness, seemed very interested in me and did talk about her problems. I then suggested she call for a doctor, and she did, and it was a psychiatrist, but I was surprised to find that her problem was much more serious than I had realized. Her first psychotic episode was in the year 1977, after which she came very close to an actuary, but that was it. At that time we could not diagnose or treat psychotic disorders effectively. But the woman had to be hospitalized in a state psychiatric treatment center, and despite having a very difficult time in this clinic she was very able to come out much stronger and be accepted by others as a normal person again. She was also very much open and honest to everyone, and it was evident that she was trying to live her own life. The psychiatrist prescribed her antipsychotic medication plus a drug to be taken several times daily, to prevent her paranoia. After a few months we realized that this drug was not working correctly without a significant dose, and we tried in vain to determine the cause of her difficulty and make her take the correct dose of the antipsychotic medication.
