PATIENT AND METHOD: A 47-year-old female patient with no known risk factors presented with severe pain in the maxillary central incisor. Patient's symptoms were not relieved by conventional treatment. The patient had also suffered a fracture within the same incisor several years ago. Her surgeon recommended a case de Anomalies en Orders. A series of examination were conducted: x-rays were normal; ultrasound was normal; and the patient was treated with a full maturation of the canine root canal. Two weeks later following a complete maturation of the canine root system, a recurrence of pain was found in the tooth; treatment was undertaken again. After further recurrence and failure to improve treatment was not viable. Patient had undergone an emergency operation and was on bed rest for 2 weeks. One month following the surgery, the recurrence of pain in the maxilla was found and the dentist removed the impacted bone to the right. An incisor root was reattached to the adjacent incisor. On follow-up two days later, she reported normal surgical results with no complications. CONCLUSIONS: A maxillary central incisor with two root canal canals is unusual in the teeth of this patient. Treatment was successful because her dentist followed a thorough cleaning and shaping of the root system and removal of all remnants of adnexal tissue from the dental surface, which he believed were causing the problem.
