Your Response is valid for one year from the date it is received by CSE (except where the CSE Act requires that it is for a period longer). All letters shall be treated in the same way as confidential, privileged and other documents (as defined in the Rules of Court, section 761.05) unless otherwise indicated. If you do not follow the information on this form or do not use this form, write a letter to: Insurance Company CSE. If you do not submit a Notice to Party (as defined in the Rules of Court, section 761.13) with your application, your application will be considered as withdrawn, and you have no further opportunity to submit an application for that dispute resolution service. If after you have complied with your obligations as set out in this Arbitration form, the CSE finds no reason to reconsider, cancel your coverage or suspend or terminate your policy for the time period covered by the cancellation or suspension, your policy will continue for the same period of time.
