It was discovered that the ice was about 500 meters thick and that there was a large area of open water. This was not anticipated in the fishery and the captain stated that the ice was expected to last through the fishing season. The ship was approaching the Fisk in a northeasterly direction, when an ice block broke and grounded the bow of the ship and a large quantity of the ice collapsed under the vessel. The Captain was not aware that the ice was unstable, which he stated led him to believe that the ship was in stable condition and therefore not in danger. No icebreaker was in harbor during this time. There were two persons located on the sea ice at the time of the accident that the captain noticed and requested that they be kept out of the water, as he was unable to see to their safety. They were removed and the Captain continued on to the Fisk. As a result the ice was not taken off before entering into Halifax Harbor in accordance with industry regulations. The Investigation also revealed that the captain was not in a position to communicate with and instruct the crew. The vessel was sailing on ice and his safety instructions were directed from a radio transmission on board of the vessel. A review of the ice by the U.S. Coast Guard confirmed that it was still stable, but the captain's instructions should have been communicated. The ice was removed from the icebreakers and the ice was hauled off, however on the way to the harbor the crew stopped in a small Fisk called the Old Harbor and the ship continued on ice, when the ice broke open again and the ice struck onshore and grounded into the sea at the end of the pier. During this investigation the investigation officer(s)/engineer(s) determined that there were multiple impacts and each of these impacts resulted in a collapse of the ice, or loss of ice, onto the vessel. After investigation of each impact, and looking back over a period of 30 minutes or so, the crew realized that the first impact, when the ice broke, had caused the ice to break and the first chunk would strike the vessel at a relatively shallow angle, which is what caused the first impact to occur. The investigation determined that this event would not have occurred if the Ice Breaker had come, either before or after the fishing vessel was underway.
