The purpose of this form is to determine if the athlete is in full and proper compliance with NCAA legislation at the time of the program that the application is submitted. If the athlete is no longer in full and proper compliance at that time, then his/her eligibility will be reviewed and the athlete will be ineligible for participation until the athlete is re-informed of his/her eligibility to continue. You may not provide a reason for the athlete to not be in full compliance. If the athlete refuses to complete this form, then his/her eligibility is denied. If you fail to follow the rules to the letter and do not fulfill your obligation, then you can be penalized. You must provide your name, address, date and time of birth, phone number and a physical exam for the purpose of determining eligibility. Once your form and test results are returned, you will be notified by email to discuss your eligibility. If you do not follow the guidelines, you risk getting in trouble with the university. This includes, but is not limited to, missing work, being suspended or being academically ineligible. Athletes are allowed to participate in the athletics program in accordance with NCAA regulations that are in place for the student-athlete during the specific season they enter into the program. The athlete is expected to be eligible for competition under NCAA rules and regulations. Once the athlete is eligible to compete under NCAA regulations, then NCAA rules and regulations apply to the athlete and athlete's participation in any other activity at the university is a violation of the NCAA rules and NCAA regulations. A parent or legal guardian signing a form is not an athlete, even if the athlete is an NCAA member. The student-athlete is responsible for the consequences of his/her act, regardless of whether they are the parents or legal guardian of the athlete. This includes but is not limited to, violations of the university-law by the parent/legal guardian to the student-athlete.
