This is the website where all the information regarding the Health Registry Standard is located. The page is currently in the process of being updated. 2 IMPLEMENTATION INITIATIVES Here you'll find information regarding the implementation of the Health Registry Standard. 3 IN A NUTSHELL The health information in the Health Registry Standard (HRS) is derived from information included in the Hospital Inpatient Registry and a Health In-Patient Database. The Hospital Inpatient Registry information includes much health information items. The Health Inpatient Database contains the demographic information of all registered patients in the Ministry of Health and is used by the Ministry to implement the HRS. In the process, certain demographics are removed because they are not necessary for the determination and reporting of health information elements in the Registry Standard. 4 METHODOLOGY AND DATA SOURCES When information must be included in the Registry Standard, it is retrieved from the Hospital Inpatient Registry through analysis of the data items contained in the HRS. In the case that demographic, health information, medical record, or clinical information is extracted from other sources, it must be converted to the format required in the regulations. 5 METHODS OF ASSESSMENT The following procedures for the evaluation of the Health Registry Standard were established by the Ministry of Health and Ministry Responsible for Seniors: 1. The HRS include information regarding all registered and non-registered patients in the Ministry of Health in the year of publication and annually thereafter for a 10-year period. Data items are assigned a value of 1 (positive) or 0 (negative). 2. The results of the Health Registry Standard assessment process are used as the basis for calculating the compliance level of the HRS. 4.1. The results of the Ministry of Health's health records and medical record audit programs are used to evaluate the compliance of the HRS. Each year from 1999 to 2006, a team of physicians, nurses, and health records specialists from Health Care Ontario, an entity wholly owned by the Province, was contracted with the Ministry of Health to evaluate the HRS. These specialists conducted an audit of the HRS. 4.2. The Ministry of Health's Health In-Patient Database, which is used for the HRS assessment, was audited by the Ministry of Health's Compliance Management and Audit Section (CMA).
