The Director or other Health Care Coordinator may also deny your request for treatment, or may refer any claims alleging fraud to the Attorney General's Office for possible prosecution. Please note: A treatment plan is not the same as a medical treatment plan and is not considered the same as medical coverage. A treatment plan cannot be used for preventive medicine. All health professionals must comply with the requirements of HIPAA. To the Applicant: You will receive information from your employer/social security number about your status. If you did not receive a letter from your employer/social security number, contact your health professional or the appropriate office of the state agency. The information will be used to prepare your treatment plan (OCF-19). If you are an employee, and you wish to file a claim in connection with a claim made under the California Workforce Compensation Act, you can contact your private insurance agent/lawyer. The information will be used to prepare your claim if it is covered under your plan or is a direct benefit of your plan. If you are a beneficiary, and you want to file a claim, you may contact your employer/social security number or the state agency who administers the program. If you decide to file a claim, the Health Care Coordinator will contact you and complete the forms and submit them to the appropriate agency. The Office of the Chief Counsel for Health Care Information will send you a copy of the form.
