If the health professional or practitioner doesn't return your copy of your treatment plan, please advise the Health Care Coordinator immediately, and be aware that if the health professional or practitioner fails to return part or all of the requested evidence when required, then your health professional may report to the Director your failure to adhere to your treatment plan within 1-month of the date he or she receives your Treatment Plan, at which time the Director may impose a penalty fee on you for non-adherence to the treatment plan. The Director or other Health Care Coordinator may also deny your request for treatment, or may refer any claims alleging fraud to the Attorney General's Office for possible prosecution. Please note: A treatment plan is not the same as a medical treatment plan and is not considered the same as medical coverage. A treatment plan cannot be used for preventive medicine. All health professionals must comply with the requirements of HIPAA. To the Applicant: You will receive information from your employer/social security number about your status. If you did not receive a letter from your employer/social security number, contact your health professional or the appropriate office of the state agency.
