Persons who do not yet have a passport may obtain one within several months either through obtaining a new passport under the Visa Waiver Program, or by obtaining an unexpired U.S. passport at the address listed in Section IV below. Section I of this application contains information that must be completed and received by the U.S. Consular Officer within 90 days of application, and it provides a checklist of items the applicant may wish to discuss with the consular officer at the time of the interview. Section II of this application contains information that should be completed at the interview. Section III outlines other required information, documents, and/or information the applicant should consider before submitting the application. For some information not outlined in Section II of this application, or items required at the interview, a completed Form DS-2019 will be required from all applicants. A copy of that form should be provided to the U.S. Consular Officer upon request.
