No. has been transferred shall not be treated as the account of the transferor of the accounts described in paragraphs (b) and (d) of this section unless the transferor furnishes to the Office of the State Tax Commissioner: (1) All the following: (i) The names and addresses, as well as all Tins issued by the IRS, of the person to whom it was transferred. (ii) A listing of the accounts from which the Account was transferred. (iii) The number of the corresponding Form 1099-DIV. (2) If the Account was received from one of the following sources: (i) The executor or administrator of the deceased s.
