You are required to provide a completed application form which you may request in person, by mail, or by phone to any of the offices listed below: The applicant's doctor at any area hospital, OR the applicant's doctor's office or the patient's physician at any general doctor's office within their practice area. The applicant's attorney at any area law office. The applicant's employer. The applicant. 1D. All applicants must complete the application for all forms of body art. The form must be provided for the applicant at the time the application is submitted to DEKALB COUNTY BOARD OF HEALTH No. 01-00013 or at time of service for a hearing. This form will allow the public health officer to conduct a background investigation prior to making any determination of the validity or otherwise of the application. Please ensure that you include all required information on the application. 1E. If an applicant is the holder of a valid professional licenses, their employer shall notify DEKALB COUNTY BOARD OF HEALTH No. 01-00013 of all required information contained on the background investigation form. This information may be accessed at any time and the information provided may be used in determining the validity of the license.
