The deadline to complete this form is September 25, the form is due September 30, and your proposal must be submitted by November 30, to be eligible for award. All student proposals must include their undergraduate major/minor/certificate. Students must complete the major/minor/certificate proposal. Undergraduate majors have the lowest priority, if there is no undergraduate major associated with an undergraduate minor. In cases of conflicting majors, the undergraduate major is the highest priority. If there is a major associated with a student's non-academic program, the Office of the Registrar will review all proposals for conflict.
