In 2004, the most recent year for which data are available, an estimated 1,300 railroad workers died on the job due to injury.2,3 Nearly one in six, or 16%, of railroad employees admitted that they had been diagnosed with asthma on the job, and 1 in 5, or 17%, reported that they had developed a respiratory infection such as chronic bronchitis on the job. The average age of railroad employees who developed asthma on the job was 51 years, and over 1 in 3, or 39%, reported that their employers had refused to allow them to use a regular work inhaler because of their disease.5 Because of its ability to save lives, the safety critical rail worker (SCAR) approach seeks to recognize the need to address the physical and mental risks associated with a job that involves the manipulation of hazardous, potentially-dislodged, or dangerous equipment. The most recent Federal Railroad Administration (FRA) standards specifically address the risks associated with smoke inhalation.5,6 The goal of the SCAR approach is to protect workers from the hazards associated with the work environment by requiring the employer to provide their employees or job applicants with a medical screening form that contains information for the worker to answer before the work day begins. The form is administered at a workplace health and safety facility or by telephoning the employee. It is important that any screening form be completed by someone other than the employee prior to the start of the work day. As a condition of employment, or to obtain special privileges, the employee must consent to allowing the employer and its representatives to test his or her medical condition.
