The NICE guidelines for the diagnosis of Pneumococcal Disease and the diagnosis of Pneumococcal Infection are set out in separate NICE documents (13). Pneumococcal geologic testing is not a routine part of routine health care and is the subject of the ongoing debate whether this testing is of value in primary or secondary prevention (14). NICE has recommended that screening at the point of care be expanded and should be combined with pneumococcal geologic testing in selected settings including at the time of first exposure to a person with Pneumococcal Disease (14). There are different schools of thought on why a geologic test is still considered necessary even though geologic testing is not considered helpful for the diagnosis of Pneumococcal Disease by NICE (15–17). Most studies agree that NICE may have considered positive serology evidence sufficient to determine a presumptive diagnosis of Pneumococcal Disease, but this has never been directly tested to be the case (14, 18). The main debate over the benefit of geologic testing is whether geologic testing is informative and useful for the diagnosis of specific Pneumococcal Diseases (PDD) (such as Streptococcus pneumonia, Streptococcus galactic [and, in the U.K., Bordetella pertussis], Nasser meningitis, Hemophilia influenza type 1 and type 2 [Hib2]). Although geologic testing has been shown to reduce the risk of developing PDS, several recent studies have questioned this benefit (19). The aim of this review is to review the current evidence on the utility of serological testing, and, if present, the quality of the evidence (20; 21). Geologic testing is important for Pneumococcal Disease and should be performed periodically to detect recurrent or new cases.
