For a schedule showing the premium amount for one individual, use Table 1 to find the premium amount for all individuals in the same family. Use Table 2 to find the premium amount for individual members of a couple. Enter the premium amount for yourself and your dependents as their share of the premium amount of the family or the amount paid by your spouse or a dependent listed in column A of the appropriate Table. Enter the premium amount for dependent members in column B.
