In a research paradigm that allows the evaluation of the interplay between design and archetype, we examined the process of design archetypes and the design of patient-specific archetypal patterns. Results indicate the potential of these patterns along with design archetypal patterns to guide the development of an ontology in health information systems (Hiss). Introduction In a recent study of the design of an ontology of health information systems (Hiss) in Hiss Health System [1], it was found that each component of the health system has a role to play in developing a comprehensive model of health information. These roles are reflected in the design of the design archetypes in Health information systems [2]–[6]. These archetypal patterns are based on the integration of archetypal patterns of health. An analysis of an interactive health care application, Hiss, found that the most frequent user of the system was a patient-centered health practitioner for whom archetypes and principles of health informed the system design development [3]. Hiss is one of the largest health information systems currently deployed (4.5 billion records in the USA) [7] and the author, David Littler, believes that “The greatest challenge facing health informatics practitioners in the future is how to apply [the patterns] to the individual and the clinician” [3]. In a recent research project we aimed at analyzing archetypal patterns of health through the use of a prototyping process on an interactive patient questionnaire and design prototyping process for an on-line electronic health record (EHR) [8]. A key finding of our study was that the health care application Hiss, was able to address both the needs of health practitioners and health information users at the application level. The application was designed to ensure appropriate care using established health knowledge information systems and the design of the health knowledge systems was in fact informed by the use of health information systems concepts and principles that were based on health knowledge information [9]. Our analyses of Hiss demonstrated the potential of using design archetypal patterns to guide the development of an ontology of health information systems (Hiss).
