Determine the outcome of each case by selecting whether the injured or ill employee required days away from work, job transfer or restriction, or other recordable cases.

Calculate the total hours worked by all employees within your company during the reporting year to determine the incidence rate.

Review the completed t r i r form for accuracy and ensure that all required fields have been filled out.

Keep a copy of the completed t r i r form for your records and submit it to OSHA by the designated deadline.
