The form is used by the relatives or legal representatives of the deceased beneficiary to apply for accrued benefits which were not paid prior to death. If several individuals claim for benefits, each of them should complete a separate form. Keep in mind that the surviving spouse, children and parents of the deceased veteran should use Va 21-534 form.
What is the VA 21-601 for?
This form is an application for accrued payments due to a deceased beneficiary. The information provided in the form is required by the Department of Veterans Affairs to decide the sum of money that should be paid to the applicant. If the beneficiary doesn’t have any living relatives, the accrued benefits may be reimbursed to the person who paid the burial expenses or expenses for the last illness of the beneficiary.
Is the VA 21-601 accompanied by other forms?
The applicant must attach a copy of death certificate. If there is a need, the copy of the letters of administration or letters testamentary, bills for burial expenses and expenses for the last illness should be included.
