How to Become a New Member
The process of becoming a new member of the Providence Health Plan depends on when you made your application and if you are a non-resident of Rhode Island. For New Members The application for new members is available at . To learn about the benefits that we offer, apply online or for more information about becoming a new member, contact the Customer Service Desk:, or (toll-free), from 8:30 a.m. until 7:00 p.m. Monday through Friday. Our call center will be open the following Monday (November 1), from 8:30 a.m. until 7:00 p.m. Your application will be processed in the order received. You must be a resident of Rhode Island to become a member of the Providence Health Plan.
New Members Applicants in New England
New members applying for health care services under the Providence Health Plan must be a resident of the State of Rhode Island and must be in the Age 60 years of age or older than of March 28, 2018. Applicants applying in New England may be eligible for a lower rate. Applicants under the age of 25 may be admitted at the full rate. New Members Applicants in New York
Applicants for new members who are residents of New York must meet one of the following criteria to be eligible: be a New York state resident or be an American citizen who is living abroad (in foreign countries) and is not a non-resident of New York and who has an address in New York State (at the time of enrollment)
be a New York citizen who has a residence outside of New York State (at the time of enrollment)
have an address in Pennsylvania or New Jersey (at the time of enrollment)
have an address in Connecticut (at the time of enrollment) and are over the age of 30
Have a New York address (at the time of enrollment) at the time of their application
Applicants under the age of 25 may be admitted at the full rate.
